ANGINA PECTORIA

DR. KRISHNA PRASAD
DEPARTMENT OF PRACTICE OF MEDICINE
Reader in Dr. R.B.S.G.H.M.C.H. Amwan,Bodhgaya

Angina pectoris derives from Latin and translated as tight chest. Actually the name “Angina pectoris” is the name for a clinical syndrome rather than a disease. The pain and discomfort may be due to myocardial ismia sclerosis of the coronary asterisis. Myocardial ischemia is caused by an imbalance between myocardial blood supply and oxygen demand. Angina is a common presenting symptom(typically, chest, pain.) among patients with coronary artery disease. Angina is made worse by factor which increases the oxygen demand in the myocardium. A low aortic diastolic pressure may contribute to an insufficient diastolic filling of coronary arteries.

CLINICAL SIGNS;-

Typical angina is uncomfortable pressure, fullness, squeezing or pain in the centre of the chest . The discomfort also may be felt in the neck, jaw, shoulder, back or arm. Many types of chest discomfort aren’t related to angina. Acid reflux (heartburn) and lung infection or inflammation are examples, There is a sensation of oppression or tightness in the middle of chest-‘like a band around the chest. ’when describing it patient usually put their hands with clenched fist on the sternum. The pain is aggravated after extortions and ameliorated after rest, lasting only for a few minutes. The pain is also experienced by walking against the wind. The pain may often extend to the neck and jaws. The pulse is of high amplitude characterized by typical water hammer pulse resulting from regurgitation of blood back into the left ventricle.

CLINICAL FINDINGS:-

ECG it may remain normal for many years but ultimately evidence of left ventricular hypertrophy with tall T waves is seen. It Characterized arrhythmia and dysrhythmia. A test that is given while a monitored. A stress test may be used to detect coronary artery disease, and/or to determine safe levels of exercise following a heart attack or heart surgery. Phonocardiograms: This reveals the presence of aortic diastolic murmurs which starts immediately after the aortic component of second sound. It is diminendo in shape. A progressive decreases in this myocardial functional impairment.

Diagnosing, Management & Treatment of Angina pectctoris: In addition to a complete medical history and medical examination, a physician can often diagnose angina pectoris by noting the patient symptoms and how/when they occur. This is also important from our Homoeopathic point of view where the case are trated on the basis if totally of symptoms. The past history d be taken into account as these can be valuable in ascertaining the true picture of the diseases, and the most important of all observations include the mental generals of the patient-his mental setup and attitudes, his behavier aspect as well a s the expressions of his sub conscious of and unconscious mind. While consulting the repertory after proper case taking and perceiving the true picture of disease and consultation with the repertory, individualisation can be done.
